Your Medical History

Effective treatment for any medical condition begins with an accurate and thorough medical history. Physicians use the answers to these questions to identify sign and symptoms that suggest the presence of disease, like obstructive sleep apnea.

The American Sleep Apnea Association is working with Arbor Medicus™ to provide people who think they may sleep apnea with a web-based program to compile their medical history to share with their physician to determine whether testing is necessary.

The Arbor Medicus™ program has a feature to allow your physician to review your medical history record without needing to print it for them.

You can also use the program to follow your progress from diagnosis through treatment by returning to provide follow up information.

Use this link to enter the program or copy and paste this link into your Internet Explorer browser - http://www.arbormedicus.com/chart/.
(Note: The program requires Microsoft Internet Explorer 5.5 or higher and the Adobe Flash player in order to work properly.)

Once you register you can start and stop as often as you need in order to finish entering the information.

Remember you can add your primary care physician and specialist so they can see the information without you needing to print it out.

A complete medical history is the first step to determining whether a diagnosis of sleep apnea is appropriate.
